2. Please print name and all information clearly. |

3. Choose Men’s or Ladies style -SELECT YOUR SIZE with an (X) - write in $ amount in the right hand column. |

4. The shirt will be Black with yellow side panels. Will include a VFW or VFW AUX. embroidered on the left side of the shirt. |
||||||||

5. Due to the sport style of the shirt material there will be NO left hand pocket. |
||||||||
|
6. The Ladies shirts will be "v'' neck. |
||||||||
|
7. If you wish to get additional embroidering you will need to get that on your own. |
